Write down the basic questions you would need to ask any athlete. Some basic questions include name, age, birth date, occupation, employer, height, weight, years playing the sport, division, previous team(s), birth place, and current place of residence.
2Add questions specific to the sport. For example, ask what position the athlete plays if they are a football or baseball player. If you are writing a survey for a hockey player, find out if they shoot right or left. For track and field athletes, find out in what specific event or events they are participating.

3Ask the athlete personal questions. Be careful not to get too personal, but it is fine to ask them about favorite pre-game meals, favorite place in town, family, nickname, and what they do in the off-season.
4Include a section for the athlete to fill in their career highlights. In this section, ask questions about records the athlete made in his career. For example, in football, ask the athlete to fill in the record number of touchdowns in a season, or number of completed passes in a season.
5Proofread the final copy of the biography survey and make sure the questions are easy to understand and are not too personal. Include a disclaimer that states the athlete need not include information they find offensive or too personal. Let the athlete know where this information will be distributed and who will have access to the biography information.
